The new Kentucky Certificate of Death is now available for use! We received approval from the Kentucky Dept. of Vital Statistics for our new electronically-fillable death certificate form. To access this new form, just log into your account, go to Advanced Settings and click Create a Kentucky Certificate of Death. Or click the button within the New Obituary screen.
Please be sure to print the form at 100% with no page scaling or rotation!
